We use OTRS, which is pretty decent (the best free tracker we could find), but its lacking quite a few features we'd like. We will soon be moving to the the Drupal Issue Tracker module, and customizing it to suit our needs. If you have a developer that can do that, its a good way to get an already decent issue tracker into one perfect for your needs.
