In a non-surgical modification of the nose, a hyaluronic filler (Restylane usually) is injected to the root to reduce the apparent height of the hump. If the hump is small enough this fill can eliminate it. The tip can be lifted slightly also by filler placed at the desired high point of the tip.

The advantages are obvious: no surgery, no downtime; hundreds, not thousands of dollars. It is often a good preliminary step before rhinoplasty in appropriate cases.

The disadvantages: it is temporary. The nasal bones nor the tip can be narrowed by this technique. Additionally, injecting these materials are not without risk. An injection into a blood vessel can result in (rarely) devastating tissue loss. Lack of basic sterile technique can result in lumpiness (granulomas).

In general there are two types of rhinoplasty, open and closed. The closed rhinoplasty has incisions only on the inside of the nose, the classic type of rhinoplasty originated by Joseph in the 30’s. It also may include resection of the nostril sills to make the nostrils smaller and reduce their width. This will leave a small, but usually an unnoticeable scar along the nostril sill. The other type of Rhinoplasty is the open rhinoplasty. A V-shaped incision is made in the nasal columella and then continued internal allowing the nasal tip to braised and the cartilages directly visualized for trimming, suturing, and or grafting. For me this worked best for a complicated nasal tip, especially one that required grafting to correct asymmetry. The new frame work for the tip could be treated and observed directly. When closing, fine sutures are used and the scar usually heals very well and is very hard to see.
