Asphyxia - It is a condition caused by interference with respiration or due to lack of oxygen, which may cause unconsciousness & Death.
Classification of Asphyxia
- Pathological
- Toxic
- Environmental
- Traumatic
- Positional
- Iatrogenic
- Mechanical
Mechanical Asphyxia
- In this type, Air-passage is blocked mechanically
It is divided into 2 periods
- Pre-asphyxial period
- It is a delay in breath
- A person without any harm can delay breath for 10–15 seconds.
- If the obstacle is not removed between 10–15 seconds, the next period develops.
- Asphyxial period (It has 4 stages)
- Stage of Dyspnea with convulsions
- It is expressed by enhanced deep inspiration at which organism tries to compensate lack of oxygen.
- Stage of Relative Rest
- Short respiratory stand-still due to depression of functions of Respiratory center.
- Stage of Terminal breath
- Appears at 4th or 5th minute, when deep breath and pauses develops.
- Terminal stage
- It is complete respiratory standstill. Heart can function within 5–30 minutes.
Classification of Mechanical Asphyxia
- Strangulation (external pressure on neck)
- Hanging
- Ligature strangulation
- Strangulation by hands
- Obstruction of external respiratory airways
- Closure of external respiratory orifices (Ex : Smothering)
- Closure of airways by impactation of foreign bodies (Ex : Chocking)
- Closure of airways by fluid (Ex : Drowning)
- External compression of chest/abdomen
- Insufficiency of oxygen in inspired air.
Pathology of Asphyxia
- Asphyxia
- Capillary dilation
- Stasis
- Reduced venous return to heart
- Reduced pulmonary blood flow
- Deficient oxygenation
- Further Asphyxia
Hanging
- It is a form of Asphyxia which is caused by suspension of body by a ligature (loop), which encircles the neck, the constricting force being weight of the body.
Classification of Ligatures (Loops)
- According to material
- Hard (Metallic chains & Wires)
- Semi hard (Leather, belt)
- Soft (Saree, bed-sheet)
- According to device of knot
- Running
- Fixed
- Depending upon number of turns of the ligature around the neck
- Single
- Double
- Multiple
Causes of death at Hanging
- Asphyxia
- The constricting force of ligature causes compression, narrowing of lumens of larynx and trachea
- It forces up the root of tongue against the posterior wall of Pharynx
- And folds the epiglottis over the entrance of larynx to block the airway
- Venous Congestion
- The jugular veins are blocked by compression of ligature
- It results in stoppage of cerebral circulation
- Combined asphyxia and venous congestion
- Cerebral anemia
- Pressure on large arteries on the neck produces cerebral anemia and coma
- Reflux vagal inhibition
- From pressure on vagal sheath or caratoid bodies
- Fracture of cervical vertebrae.
Post-Mortem Appearances in Hanging
- External apperances
- Neck is stretched due to upward pull of ligature
- Head is always inclined to the side opposite to the knot due to gravitational forces.
- Face is pale
- Eyeballs appear prominent due to congestion
- Tongue is protrude due to pressure by ligature at its base
- Bloody mucus can escape from mouth and nostrils.
- Ligature Mark
- It is imprint of loop on skin of neck
- It is pale in color, later becomes yellowish
- Ligature mark usually encircles the entire neck, except for place, where knot was located.
- Mark is located above the level of thyroid cartilage.
- Character of ligature mark depends on :
- Nature of ligature (Thin rope or saree)
- Tightness of encircling ligature
- Number of turns of ligature around the neck
- weight of the body
- Length of time body has been suspended.
